  • Your questionnaire responses are kept confidential according to the policies of the University of California San Francisco's Committee on Human Subjects Research (www.research.ucsf.edu/chr/). This policy ensures that ONLY researchers that are directly involved in this study or the study sponsor will have access to any personally identifiable information. We will NOT share your contact information or responses with other third parties without your permission.

For participants in the research study, study coordinators will review a separate privacy policy with you prior to enrollment.

What if I am not eligible for this research study?
We will keep your questionnaire responses on file. If we identify a future stroke study that you may qualify for, we will contact you using the information that you provide in the questionnaire.
